Financial Information

Our doctor and staff are proud to be a team whose primary mission is to deliver the finest and most comprehensive periodontal services available today. We are concerned about your dental care and we want to ensure that it is performed in the most responsible manner. In order to assist you with the investment in your dental health, we have outlined our payment policy.

Payment is due in full at the time services are performed. We will file your claim to your dental insurance company for your direct reimbursement of any possible covered charges, however, you are responsible for paying the full charges on the day of service. 

Please note that in some situations payment may be required in advance of treatment and will be determined on an individual basis. All arrangements other than full payment at the time of service must be made in advance of your appointment.

For treatment in excess of $500.00, we can file a Pre-Determination of Benefits for you at your request, however, because a Pre-Determination is only an estimate of benefits from your insurance company based on your plan and eligibility, any balance remaining after your insurance company has paid their benefit portion will be your responsibility. Final reimbursement by your insurance company may vary due to plan types, insurance maximums, deductibles, and co-payments and exclusions. Please remember that we file with your insurance as a courtesy to you, and any dispute concerning final payment by your insurance carrier should be directed to your insurance company for resolution.

Emergency/Single-Visit Payments

For single-visit procedures, our policy is that payment is due in full at the time of service. For your convenience we accept cash, checks, money orders, and credit card payment (MasterCard, VISA, American Express, and Discover Card).

Multiple Visit Payment Options

Option 1:

Cash / Check / Money Order / VISA / Mastercard / American Express / Discover Card

Selection of this option involves payment for services and treatment rendered at your visit. When insurance is involved, we will be happy to file your insurance claim for you for direct reimbursement to you for any covered charges based on your estimated coverage. Option 2:

Care Credit

For those that would prefer an extended payment plan, our office works with Care Credit, a healthcare financing company that makes having a healthy smile affordable for almost everyone. You can easily apply for financing for your dental procedure through the link below. For treatment plans with out-of-pocket costs of $1000.00 or more, Care Credit can provide you with the monthly payment and deferred interest options that you might be looking for.

Option 3:

Health Savings and Flexible Savings Accounts (HSA/FSA)

For your convenience, we accept HSA and FSA payments made with cash, personal check, MasterCard, VISA, Discover Card and American Express. We are happy to provide you with a receipt upon request for you to obtain personal reimbursement to your HSA/FSA account.
