Insurance Benefits

Our insurance coordinator will be happy to assist you with filing your dental claims. Although we do not participate as a provider with any dental plan providers including Medicare, Medicaid and CHIP, most traditional plans will reimburse you for our services at the “out-of-network”  rates based on your specific plan benefits.

Our insurance coordinators deal with many different insurance companies. Some companies offer many different dental and medical plans. These companies can change benefits, co-pays, and deductibles many times throughout the year. We do our best to provide you with accurate coverage estimates based on information available to us from your insurance carrier. At times, it is almost impossible to accurately estimate a patient’s insurance co-payment. Many insurance companies will not give out fees until after the treatment is completed. Dealing with these companies can be difficult and time consuming. As a courtesy, we ask that you keep us informed of any change to your insurance. It is important that all information about you and your insurance is current.

For services under $500.00, payment in full is required at the time of service. We will submit a completed dental claim form for you to your insurance company in order for them to send any eligible benefits reimbursement directly to you.

If services exceed $500.00, we can submit a Predetermination (pre-treatment estimate) to your insurance company at your request based on your individual treatment plan. Your insurance will then inform you and our office of benefits available and what out of pocket costs will be. Patients are responsible for paying any charges not covered by insurance at the time of service. We will then submit a claim to the insurance company for the benefit portion.

Please be aware that a Predetermination estimate is not a guarantee of payment, and the patient is responsible for all charges incurred. Final reimbursement by your insurance company may vary due to insurance maximums, deductibles, and co-payments and exclusions. While we do our best to give the most accurate estimate for services as possible, you are still responsible for payment of any services rendered that are not paid by your insurance carrier. Also, please remember that we file with your insurance as a courtesy to you, and any dispute concerning final payment by your insurance carrier should be directed to the insurance company.

To help us process your insurance claims as quickly and efficiently as possible, please provide the office with your most current dental insurance information including the name, birth date and ID number of the insured party, employer, insurance company with mailing address, group number and 800 telephone numbers. We realize insurance coverage can change yearly, so it is important that you keep us informed of any changes.
